Account randomly locks out

Enviroment:
Windows 2000 server SP4, DC with AD and DHCP
Windows 2000 server SP4 with Exchange 5.5
Windows 2000 Pro workstation SP4, Office 2003
Issue:
At random times, sometimes while logged on, sometimes while not, my user
account locks out. The user account is not an Admin account. While logged
on, I find out when Outlook asks for my password. To fix the issue I have to
log onto the DC and uncheck the Account is locked out option under the
Account tab. At any other time this option is greyed out. I have already
eliminated the chance that someone is playing games (this sometimes happens
in the middle of the night when no one else is even awake or logged in). Any
idea what is going on and how to correct the issue? This seems limited to my
user account, although it has happend once to another user (same workstation
setup.)

Do you have an application that logs in automatically? This almost sounds
like an excessive number of invalid logon attempts. If your the admin, you
could test this by temporarily removing the lockout rule in AD. If it
doesn't happen, then turn it back on and let the detective work begin to
find out what is logging in incorrectly.

Also, As Andrei points out, there will be an Event Log entry if security
logging is on and this is what is happening....
