My Access Database appears to randomly be deleting
relationships between tables. Does anyone know what may
cause this? This has happened in the past and I have had
to completely rebuild all relationships.

Often relationships will disappear from the Relationships window even
though they still exist - I've seen this happen in my own databases
and often here. Are the Relationships themselves actually being
dropped? When you create a Query with the two related tables, does the
predefined relationship come up as the default?

I'd suggest Compacting your database for starters; you may want to do
the more drastic step of creating a new, empty database and Importing
all the objects into it. Possibly you have some subtle systems table
corruption.
