Access form being emailed and recipient fills in email form and sends back.

I have a Word 2003 Merge document to an Access 2003 database. This document
is emailed. What I would like to do is take the document and make it a form
so recipients can fill in incomplete fields. I would like it to be returned,
email, so that it can be entered back into the database. Any ideas?
 
It's far easier to craft a form in Excel and email it out. Then when the
Excel file is returned, import the data into Access.
