Microsoft,

I use a big sheet of paper (A2 size), a pencil, and an eraser.

Depends on how much money you want to spend.
I use:
Tabloid sized sheets of paper
Excel - One table per work sheet - allows entry of sample data to show
client
Font colors: Black for primary keys, blue for attributes,
red for associations/foreign keys
Visio - This can generate an access database
ErWin - Try to find one of the earlier versions before they got heavy
into reverse engineering.
Enterprise Architect see it at
http://www.netti.hu/doc/sparxsystems/ea.htm I think it is freeware.

How do you use Visio to create a database? I can't seem to figure out how to get it to export the table definitions and relationships...

You need the Pro version -- it comes with a built in template for creating
Jet and SQL Server schemas and exporting them.
