More seriously: this is a peer-support newsgroup. We'll be glad to
answer specific questions about problems you're having with Access. If
you want ideas about how to set up such a database, there is a
"Contacts" sample database provided by Microsoft; or you might want to
post a description of what's needed in
microsoft.public.access.tablesdbdesign for help with designing the
initial database. This Forms group would be useful later when you have
the basic database structure built and you need to enter and edit data
in it.

I think maybe this is meant as a suggestion for a new database template
that Microsoft might supply. If so, you should know that, no matter
what the web page you posted from may have led you to believe, this
isn't the right forum for posting suggestions to Microsoft. This is a
peer-support newsgroup for questions involving Access forms. Microsoft
personnel don't actually staff these newsgroups and accept suggestions.
