I have created a report with sub-totals of two categories, amex and visa,
which are grouped. How can I capture the sub-total separately, grouped, to
create a grand total combining the total of amex and visa together?

Use a report header/footer text box with an expression like:
=Sum(amount)
which should be the exact same expression that you used in
the group header/footer.
