Anyone have any problems with the USB ports on this board? For some reason
ports 3 & 4 have decided to stop working on my 2 month old board?

Check the USBPWR_34 header. It is near the PS/2 ports and
USB3/USB4. Make sure there is a jumper on it. Pull the
jumper, and reinsert it, to try to get a fresh contact.
That is what powers those USB ports, so if the USB device
LEDs don't light, that could be the reason.
If the ports appear to have power, check the Device Manager,
to see if there are enough entries. You might want to delete
all the USB entries in Safe Mode, and let the system
rediscover them. Sometimes the system "remembers" something
about the state of the ports that stops them from working,
and deleting the ports will flush out the registry.
