A data entry form

any suggestions please on setting up a data entry form to record credit card
transactions. It needs to be printed and later authorised etc.

This is an extremely broad and nonspecific question to ask in a newsgroup.

Are you asking what type of data should be on this form? How to display it
on the form (use subforms in some way, for example)? What types of data do
you want to store in your tables? Which data items will be available to the
user? How will the form be used by the user (while on the telephone with
someone?; doing data entry at a later time?)?
What have you tried so far? What is or isn't working, and what do you want
to do that you're having difficulty achieving?

Note that forms should not be printed; that is the purpose of reports. Forms
are for viewing/entering/editing data; reports are for printing/viewing the
results of queries on the data. I think you will want to print a report that
will be authorized.

Please provide more specific info and questions, and someone will be provide
suggestions and ideas.
