6600GT green block issue

I have installed a leadtek 6600GT 128MB RAM with forceware 71.89
drivers. When playing games i occasionally get random green blocks
making the game unplayable. A good exapmle is when you turn on night
vision on far cry. Anyone had this before? Any ideas?
My system is as follows:

P4 3.2
1GB Ram
Asus P5AD2 Premium M/B

Is your video card overclocked?
Have you made any modifications to the card (new heatsink and fan)?
Does it happen in most games?
 
Videocard not overclocked. I insatlled a better heatsink+fan (Zalmann
ones)from new, i never used the origional one. It happens in all games
to various extents. Thanks

Try patching farcry if applicable.
Try a different video card driver- always try an older one
when a current-gen driver "might" be a problem. In this
case, try a 6x,xx series Detonator

See if pointing a fan at the card helps- in which case it
may be too hot.

Check power supply voltages.

Anything similar happen in other games?
 
Sounds like your VC memory isn't getting enough cooling.
Or, worst case scenario, you damaged the card when you replaced the HS+F.
